How to Complete an Accident Report Form

All injuries and accidents must be recorded on an Accident / Incident Report Form. Ensure the report is detailed, clear and precise. Include all facts. This could be used for legal proceedings in the future.

Keeping records of accidents and incidents is required by law and is an important part of any risk management process. Analysing the accident records is a valuable management tool that can be used to aid risk assessment and put in place preventive safety actions.

Records are traditionally kept in a paper format using an accident book however, accident records are increasingly computerised. App’s are now a popular choice.

This report should include;

  • Contact details of the injured person and the person filling out the form
  • Details of the accident including the date, time and location
  • A description of the injury, including the type, location and severity
  • The treatment given
  • Was medical help requested?
  • A factual description of what happened
  • Further action such as hospital treatment or was the child sent home?
  • Not essential but recommended; A body map illustration clearly demonstrating the position and location of the injury
  • The signature of the person completing the form
  • Not essential but recommended; A signature from any staff members who witnessed the injury
  • The signature of the parent or guardian who is informed of the accident/ incident

    Inform parents or guardians of any accident or injury sustained by the child on the same day, or as soon as reasonably practicable, of any first-aid treatment given.
